Job summary

Intermountain Healthcare is hiring a Hospital at Home RN to coordinate and evaluate complex care delivery with advanced clinical expertise. You will lead virtual rounds, mentor staff, and collaborate with Tele Hospitalists, physicians, APPs, pharmacists, and care teams to ensure evidence-based practice and high-quality patient outcomes.

Qualifications

• Licensed RN in state of practice with compact license transfer within 60 days.
• BSN is required or must be obtained within four years of hire.
• BLS Certification for Healthcare Providers.
• Proven expert medical-surgical nursing experience.
• Clinical leadership experience (charge nurse, preceptor, or mentoring).
• Ability to coordinate complex care and lead quality improvement initiatives.
• Ability to work independently with autonomy in a remote environment.
• Strong communication, critical thinking, and problem-solving skills.

Responsibilities

• Coordinate and evaluate complex care delivery for Hospital at Home patients.
• Provide advanced clinical expertise, education, and contribute to research.
• Lead virtual rounds and support a pool of nurses and interdisciplinary teams.
• Collaborate with Tele Hospitalists, physicians, APPs, pharmacists, and care teams.
• Drive high-quality outcomes and influence care plans in real time.
