The RN administers in-home hospital-level treatments, coordinates mobile diagnostics, and supports services such as physical therapy and social work to meet patient needs.

Qualifikationen

• Must be licensed as an RN in Arkansas.
• Minimum one year of experience in an acute care setting.
• Ability to provide care to geriatric patients and communicate effectively with this population.
• Demonstrates growth and development knowledge across the life span; strong clinical judgment and critical thinking.

Aufgaben

• Provide hospital-level and transitional care in the home setting.
• Coordinate mobile diagnostics, equipment, and supportive services (PT, social work).
• Follow patients for 30 days post-discharge to prevent readmissions; coordinate care via telemedicine and other technologies.
• Manage IV medications, wound care, oxygen therapy, and other skilled nursing interventions.
• Collaborate with physicians, nurse practitioners, and the interprofessional team in a home environment.
