Responsibilities

• Monitor biometric data and respond per command center protocols.
• Conduct audio/visual patient care visits as directed by the care team.
• Coordinate care across the continuum and collaborate with multidisciplinary team.
• Promote patient safety and participate in quality improvement activities.
• Utilize technology-enabled monitoring and thorough documentation.
