Job summary

Lake Valley Hospice is seeking a Registered Nurse (RN) in Minnesota for weekend coverage with Friday 5:00 PM to Monday 8:00 AM shifts. The role offers hourly pay of $42–$45 with a triage/pager bonus and a supportive team focused on dignified end‑of‑life care.

Ideal candidates will have an accredited nursing program degree, MN RN licensure, and at least two years of RN experience, with hospice/palliative care preferred. Emphasis on collaboration with home health aides and facility staff.

Responsibilities

• Provide weekend nursing assessments and symptom management per patient care plan.
• Document promptly and relay changes to RN case manager and hospice team.
• Coordinate with hospice providers, pharmacies, facilities, and caregivers during weekend coverage.
• Promote Lake Valley Hospice services with professionalism and care.
• Provide scheduled and on-call nursing visits during assigned weekends.

Position Requirements

• Education: Graduate of an accredited nursing program.
• Licensure: Current Registered Nurse (RN) license in Minnesota.
• Experience: Minimum of two years as an RN; hospice or palliative care experience strongly preferred.
• Skills:
  • Independent work and consultation as needed.
  • Collaborating with home health aides and facility staff to support patient care.
  • Crisis intervention, grief support, and advance care planning.
  • Acceptance of diverse backgrounds, flexibility, and strong interpersonal skills.
  • Effective time management and attention to detail.

Key Responsibilities

• Provide weekend nursing assessments, symptom management, and care consistent with each patient’s individualized plan of care.
• Provide timely documentation and communicate changes in patient condition, interventions, and follow-up needs to the RN case manager and hospice team.
• Coordinate with hospice providers, pharmacies, facility partners, and caregivers to address patient needs during assigned weekend coverage.
• Promote Lake Valley Hospice services with professionalism and care.
• Provide scheduled and as‑needed nursing visits during assigned weekends, with on‑call coverage as scheduled.
