Job summary

Lake Valley Hospice in Minnesota is seeking an experienced RN to provide weekend nursing assessments, symptom management, and care aligned with each patient’s plan.

You will document promptly, communicate changes to the RN case manager, coordinate with providers, pharmacies, and caregivers, and perform on‑call weekend visits while delivering dignified end-of-life care. A MN RN license and graduation from an accredited nursing program are required, with hospice experience preferred.

Qualifications

• Graduate of an accredited nursing program.
• Current Registered Nurse (RN) license in Minnesota.
• Minimum of two years as an RN; hospice or palliative care experience strongly preferred.
• Strong interpersonal skills, time management, and attention to detail.

Responsibilities

• Provide weekend nursing assessments, symptom management, and care aligned with each patient’s individualized plan of care.
• Provide timely documentation and communicate changes in patient condition, interventions, and follow-up needs to the RN case manager and hospice team.
• Coordinate with hospice providers, pharmacies, facility partners, and caregivers to address patient needs during assigned weekend coverage.
• Promote Lake Valley Hospice services with professionalism and care.
• Provide scheduled and on‑call nursing visits during assigned weekends, with on‑call coverage as scheduled.

Position Requirements

• Education: Graduate of an accredited nursing program.
• Licensure: Current Registered Nurse (RN) license in Minnesota.

  • Collaborating with home health aides and facility staff to support patient care.
