Job summary

Superior Healthcare Management is seeking a Hospice Weekend Registered Nurse in Minnesota. You will provide weekend nursing assessments, symptom management, and care aligned with each patients plan of care while coordinating with the hospice team and caregivers.

The role requires an RN license in Minnesota, at least two years of RN experience with hospice or palliative care preferred, and a valid driver license with reliable transportation.

Qualifications

• Graduate of an accredited nursing program.
• Current Minnesota RN license.
• Minimum of two years as an RN; hospice or palliative care experience preferred.
• Valid driver license and reliable transportation.

Responsibilities

• Provide weekend nursing assessments, symptom management, and care aligned with each patients plan of care.
• Document timely and communicate changes in patient condition, interventions, and follow-up needs to the RN case manager and hospice team.
• Coordinate with hospice providers, pharmacies, facility partners, and caregivers to address patient needs during assigned weekend coverage.
• Promote Lake Valley Hospice services with professionalism and care.
• Provide scheduled and on-call nursing visits during assigned weekends, with on-call coverage as scheduled.

HOSPICE WEEKEND REGISTERED NURSE

Position Requirements

• Education: Graduate of an accredited nursing program.




• Licensure: Current Registered Nurse (RN) license in Minnesota.




• Experience: Minimum of two years as an RN; hospice or palliative care experience strongly preferred.
