Job summary

Lake Valley Hospice is seeking a Weekend Registered Nurse in Minnesota to provide patient-centered care on weekend shifts. The role focuses on symptom management, documentation, and collaboration with the hospice team to ensure high-quality end-of-life care.

Requirements include graduation from an accredited nursing program and an active Minnesota RN license, with a preference for hospice/palliative care experience. Weekend coverage and on-call hours are part of the schedule.

Responsibilities

• Provide weekend nursing assessments and symptom management per plan.

• Promote Lake Valley Hospice services with professionalism and care.
• Provide scheduled and on-call nursing visits during assigned weekends.

Position Requirements

• Education: Graduate of an accredited nursing program.


• Licensure: Current Registered Nurse (RN) license in Minnesota.


• Experience: Minimum of two years as an RN; hospice or palliative care experience strongly preferred.

Key Responsibilities

• Provide weekend nursing assessments, symptom management, and care consistent with each patient’s individualized plan of care.


• Provide timely documentation and communicate changes in patient condition, interventions, and follow-up needs to the RN case manager and hospice team.


• Coordinate with hospice providers, pharmacies, facility partners, and caregivers to address patient needs during assigned weekend coverage.


• Promote Lake Valley Hospice services with professionalism and care.


• Provide scheduled and as-needed nursing visits during assigned weekends, with on-call coverage as scheduled.

Additional Requirements

• Valid driver’s license, auto insurance, and reliable transportation.
