Job summary

Superior Hospice, part of the Home Health Care, Inc. family, is seeking a part-time Hospice Visit Nurse (RN) to provide compassionate, home-based care to patients in their homes and community settings.

You will conduct routine visits, symptom management, wound care, and patient education, collaborating with physicians, social workers, chaplains, and hospice aides. This role offers autonomy within a supportive leadership team and pathways to full-time as patient volume grows.

Qualifications

• Active Minnesota RN license.
• Hospice experience preferred or strong background in oncology, palliative care, or end-of-life care.
• Ability to work independently in a field-based role.

Responsibilities

• Provide routine and PRN hospice visits in patients' homes, assisted living, memory care, and skilled nursing communities.
• Perform comprehensive nursing assessments, symptom management, wound care, and medication education.
• Support patients and families through every stage of the hospice journey.
• Collaborate with physicians, social workers, chaplains, hospice aides, and the interdisciplinary team.
• Complete accurate and timely EMR documentation.

• Hospice experience preferred, or strong experience in oncology, palliative care, home health, ICU, or end-of-life care.
• Strong assessment, communication, and critical thinking skills.
• Ability to work independently in a field-based role.
• Reliable transportation and a valid driver's license.
