Job summary

Superior Hospice is seeking a dedicated Hospice Visit Nurse (RN) to provide compassionate, patient-centered care in patients' homes, assisted living, memory care, and skilled nursing communities. This part-time role emphasizes independence and clinical judgment while supporting families through the hospice journey.

The ideal candidate will hold an active Minnesota RN license and bring hospice, oncology, palliative care, home health, ICU, or end-of-life experience.

• Strong assessment and communication skills required.
• Ability to work independently in a field-based role.

Responsibilities

• Provide routine hospice visits in patients' homes, assisted living, memory care, and skilled nursing communities.
• Perform comprehensive nursing assessments and symptom management.
• Educate patients and families and support them through the hospice journey.
• Collaborate with physicians, social workers, chaplains, and the interdisciplinary team.
• Complete accurate and timely EMR documentation.

What You'll Do

• Support patients and families through every stage of the hospice journey.

What You'll Bring

• Active Minnesota Registered Nurse (RN) license.
  
• Hospice experience preferred, or strong experience in oncology, palliative care, home health, ICU, or end‑of‑life care .

• Ability to work independently in a field‑based role.
  
• Reliable transportation and a valid driver's license.
