Job summary

Superior Hospice, part of the Home Health Care, Inc. family, is seeking a Part-Time Hospice Visit Nurse (RN). This role offers a path to full-time as patient volume grows, with one-on-one patient care in home settings and collaboration within an interdisciplinary team.

The ideal candidate will hold a Minnesota RN license, have hospice or end-of-life care exposure, and demonstrate strong assessment, communication, and time-management skills. Reliable transportation is required.

Qualifications

• Active Minnesota RN license.
• Hospice experience preferred or exposure to home health, oncology, ICU, palliative care, med-surg.
• Excellent assessment, communication, and time-management skills.
• Ability to work independently in a field-based role.
• Reliable transportation and valid driver’s license.

Responsibilities

• Provide routine hospice visits in patients' homes, assisted living, memory care, and SNF.
• Perform nursing assessments, symptom management, and comfort-focused interventions.
• Educate and advocate for patients and families, with compassionate communication.
• Collaborate with physicians, social workers, chaplains, hospice aides, and the interdisciplinary team.
• Complete accurate, timely EMR documentation.
