Job summary

Family Home Health Network, an Affiliate of the Alden Network, provides intermittent home health, palliative, hospice, and skilled/custodial private care across Chicagoland. We seek a full-time Hospice Triage Nurse to perform after-hours assessments by phone and coordinate visits with on-call staff, while living in Illinois and occasionally visiting the office.

• Licensed RN/BSN in Illinois required.
• Hospice experience with end-of-life care mandatory.

Job description

Family Home Health Network, an Affiliate of the Alden Network, is a provider of intermittent Home Health care, palliative care, hospice care and skilled/custodial private care services throughout the Chicagoland area.

We are seeking a dedicated Full-Time Hospice Triage Nurse to provide compassionate and quality end of life care to patient and families. Our goal is to improve the quality of life, and allow patients and families to focus on relationship, emotional, and spiritual needs.

As a triage nurse, you will be responsible for handling incoming calls after hours, doing clinical assessments over the phone, providing clinical expertise and coordinating visits with the on‑call nurse(s) as needed/appropriate.

This position is Monday-Thursday 5pm to 8:30am, working from home, but applicant MUST LIVE in Illinois and be able to come into the main office on occasion.

• A valid RN/BSN license in the state of IL.
• At least one year of hospice experience is mandatory.
• Comfortable with electronic documentation of medical records – HCHB EMR experience a plus.
• Ability to manage patient care with specific knowledge and experience in bedside nursing, symptom management, crisis intervention, and family intervention.
• Demonstrate problem‑solving and decision‑making abilities.
• Ability to develop and maintain rapport with the Patient and family.
• Excellent communication skills, i.e. listening, oral and written skills while conversing with patients, families, health professionals and community agencies.
• Capacity to function independently with the licensed scope of practice.
• Must have the ability to handle stressful emergency situations calmly and effectively.
