Job summary

Family Home Health Network is seeking a dedicated Hospice Nurse in Illinois to provide start-of-care assessments and ongoing nursing support in patients' homes. The role emphasizes compassionate patient care, strong clinical skills, and teamwork with the caregiving staff.

The position is Monday–Friday, with responsibilities focused on starting care and delivering high-quality home hospice services to patients and families in the Chicagoland area.

Qualifications

• Must hold current Illinois RN licensure.
• Minimum of two years hospice experience preferred.
• Current CPR certification.
• Valid IL driver's license and auto insurance.
• HCHB EMR experience is a plus.
• Excellent clinical nursing and customer service skills.

Responsibilities

• Complete initial assessment of the patient, family, and facility/home environment to determine specific care needs.
• Monitor and regularly reevaluate the patient’s nursing needs and initiate revisions to the plan of care as status changes.
• Initiate start of care and coordinate with the care team for seamless service.
• Provide compassionate support to patients and families throughout home hospice care.

Family Home Health Network, is a provider of intermittent Home Health care, Palliative care, Hospice care and Skilled/Custodial private care services throughout the Chicagoland area.

We are seeking a dedicated Hospice Nurse with a strong commitment to quality patient care. To succeed in this role, you must possess excellent clinical nursing and customer service skills. You will be responsible for completing a detailed initial assessment of the patient, the family, and the facility/home environment to determine specific care needs, monitor and regularly re-evaluate the patients nursing needs, and initiate the appropriate plan of care as well as making necessary revisions as patient status and needs as they change.

THIS POSITION IS MONDAY-FRIDAY AND WILL PRIMARILY BE COMPLETING ALL START OF CARES.

Qualifications:

• Must possess current Illinois RN licensure

• Possess a minimum of two (2) years experience (Hospice Preferred)

• Current CPR

• Valid IL driver’s license and auto insurance policy

• HCHB EMR experience a plus

• Must possess excellent clinical nursing and customer service skills
