Job summary

Family Home Health Network in the Chicagoland area is seeking a dedicated Hospice Nurse with a strong commitment to quality patient care. You will perform an initial assessment of the patient, family and home environment to determine care needs and monitor nursing requirements.

The position requires Illinois RN licensure, CPR, a valid IL driver’s license, and EMR experience (HCHB preferred). The weekend schedule runs Friday 7am to Sunday 7pm, covering Northern Illinois/North Shore/Northwest

Qualifications

• Must possess current Illinois RN licensure.
• Minimum of two (2) years' hospice experience (preferred).
• Current CPR.
• Valid IL driver’s license and auto insurance.
• HCHB EMR experience a plus.
• Must possess excellent clinical nursing and customer service skills.

Responsibilities

• Perform initial assessment of the patient, family, and home environment to determine care needs.
• Monitor and regularly re-evaluate the patient’s nursing needs.
• Initiate and revise the plan of care as patient status changes.

Family Home Health Network, is a provider of intermittent Home Health care, Palliative care, Hospice care and Skilled/Custodial private care services throughout the Chicagoland area.

We are seeking a dedicated Hospice Nurse with a strong commitment to quality patient care. To succeed in this role, you must possess excellent clinical nursing and customer service skills. You will be responsible for completing a detailed initial assessment of the patient, the family, and the facility/home environment to determine specific care needs, monitor and regularly re-evaluate the patients nursing needs, and initiate the appropriate plan of care as well as making necessary revisions as patient status and needs as they change.

THIS POSITION IS EVERY WEEKEND FROM 7AM TO 7PM FRIDAY-SUNDAY. IT WILL COVER ALL OF NORTHERN ILLINOIS/NORTH SHORE/NORTHWEST SUBURBS.

Qualifications:

• Must possess current Illinois RN licensure
• Possess a minimum of two (2) years' experience (Hospice Preferred)
