Job summary

Family Home Health Network in the Chicagoland area is seeking a dedicated Home Health nurse with a strong commitment to quality patient care. You will perform a detailed initial assessment of the patient, family, and home environment to determine specific home health care needs, monitor nursing status, and initiate the plan of care as patient status changes.

Requires current Illinois RN licensure, CPR, a valid IL driver’s license with auto insurance, and preferably Home Health experience; HCHB

Qualifications

• Must possess current Illinois RN licensure.
• Home Health experience preferred.
• Current CPR.
• Valid IL driver’s license and auto insurance policy.
• HCHB EMR experience a plus.
• Must possess excellent clinical nursing and customer service skills.

Responsibilities

• Complete a detailed initial assessment of the patient, the family, and the home environment to determine specific home health care needs.
• Monitor and regularly reevaluate the patients' nursing needs.
• Initiate the appropriate plan of care and revise as patient status changes.

Job description

Family Home Health Network, an Affiliate of the Alden Network, is a provider of intermittent Home Health care, palliative care, hospice care, and skilled/custodial private care services throughout the Chicagoland area.

We are seeking a dedicated Home Health nurse with a strong commitment to quality patient care. To succeed in this role, you must possess excellent clinical nursing and customer service skills. You will be responsible for completing a detailed initial assessment of the patient, the family, and the home environment to determine specific home health care needs, monitor and regularly re-evaluate the patients' nursing needs, and initiate the appropriate plan of care as well as making necessary revisions as patient status and needs as they change.

Minimum Qualifications:

• Must possess current Illinois RN licensure
• Home Health experience preferred
• Current CPR
• Valid IL driver’s license and auto insurance policy
• HCHB EMR experience a plus
• Must possess excellent clinical nursing and customer service skills
