Job summary

Family Home Health Network is seeking a dedicated Hospice Nurse to deliver high-quality care. You will perform detailed initial assessments of patients, families and care environments, and regularly re-evaluate nursing needs to guide and adjust the care plan.

The position requires weekend shifts (Fri–Sun, 7am–7pm) serving Northern Illinois areas north of Chicago. Robust nursing and customer service skills are essential for success in this dynamic team.

Qualifications

• Must possess current Illinois RN licensure.
• Minimum of two years' hospice experience preferred.
• Current CPR certification required.
• Valid IL driver’s license and auto insurance.
• HCHB EMR experience is a plus.

Responsibilities

• Perform a detailed initial assessment of patient, family and care environment.
• Monitor and re-evaluate nursing needs and revise the plan of care as status changes.
• Provide high-quality nursing care and exceptional customer service.
• Work weekends: Friday–Sunday, 7am–7pm, covering Northern Illinois/North Shore/Northwest Suburbs.

Family Home Health Network, is a provider of intermittent Home Health care, Palliative care, Hospice care and Skilled/Custodial private care services throughout the Chicagoland area.

We are seeking a dedicated Hospice Nurse with a strong commitment to quality patient care. To succeed in this role, you must possess excellent clinical nursing and customer service skills. You will be responsible for completing a detailed initial assessment of the patient, the family, and the facility/home environment to determine specific care needs, monitor and regularly re-evaluate the patients nursing needs, and initiate the appropriate plan of care as well as making necessary revisions as patient status and needs as they change.

THIS POSITION IS EVERY WEEKEND FROM 7AM TO 7PM FRIDAY-SUNDAY. IT WILL COVER ALL OF NORTHERN ILLINOIS/NORTH SHORE/NORTHWEST SUBURBS.

Qualifications:

• Must possess current Illinois RN licensure
• Possess a minimum of two (2) years' experience (Hospice Preferred)
• Current CPR
• Valid IL driver’s license and auto insurance policy
• HCHB EMR experience a plus
• Must possess excellent clinical nursing and customer service skills
