Job summary

Family Home Health Network, an Affiliate of the Alden Network, provides intermittent home health, palliative, hospice, and skilled/custodial private care across Chicagoland. We seek a full-time Hospice Triage Nurse to perform after-hours assessments by phone and coordinate visits with on-call staff, while living in Illinois and occasionally visiting the office.

Qualifications

• Licensed RN/BSN in Illinois required.
• Hospice experience with end-of-life care mandatory.
• Comfort with electronic medical records (HCHB EMR a plus).
• Able to work after-hours triage and coordinate visits with on-call nurses.
• Strong communication and crisis management skills.

Responsibilities

• Handle after-hours calls and perform clinical assessments over the phone.
