Job summary

PruittHealth Hospice is seeking a Travel Nurse (RN or LPN) to provide compassionate hospice care in patients' homes and facilities across multiple Georgia locations. This role requires willingness to travel to Augusta, Cordele, Macon, Savannah, Swainsboro, Union City, and Valdosta areas, with top‑of‑market pay and merit increases.

The ideal candidate has a valid RN or LPN license, at least 3 years of nursing experience, and exposure to palliative or home health care.

Qualifications

• Active RN or LPN licensure in the state of practice
• Minimum of three (3) years of nursing experience
• Palliative care, hospice, or home health care experience preferred
• CHPN certification preferred

Responsibilities

• Take and process palliative referrals
• Support Palliative Care Nurse Practitioner and Medical Director
• Schedule visits to support goals of care as ordered by MD
• Coordinate and communicate patient care goals among providers
• Make calls to present the palliative program and obtain consents
• Communicate assessment findings to attending physician
• Complete documentation accurately and timely
• Inform team of new admissions and discharges
• Coordinate referrals via the referral process
• Act as liaison for medical directors and nurse practitioners
• Run billing reports and address validation holds/billing errors

KEY RESPONSIBILITIES:

• Responsible for taking and processing Palliative Care referrals.
• Provide support to the Palliative Care Nurse Practitioner and Medical Director.
• Will schedule visits to support goals of care as ordered by MD.
• Coordinate and communicate patient care goals between all providers.
• Make calls to present the palliative program, obtain legal consents, and assess interdisciplinary needs.
• Ensure communication of assessment and reassessment findings to attending physician.
• Complete all documentation accurately, timely, and in accordance with company policy and procedure.
• Inform palliative team of all new admissions and discharges.
• Coordinate referrals utilizing the referral process.
• Serve as a liaison for medical directors and nurse practitioners, enhancing effectiveness and compliance.
• Run billing report and address validation holds/ billing errors

LICENSURE, CERTIFICATION, AND EDUCATIONAL REQUIREMENTS:

• Current, active, and unrestricted Registered Nurse (RN) OR Licensed Practical Nurse (LPN)licensure in the state of practice
• A minimum of three (3) years of nursing experience
• Palliative care, hospice, or home health care experience is preferred
• CHPN (Certified Hospice and Palliative Nurse) is preferred
