Job summary

PruittHealth Hospice is seeking a Licensed Practical Nurse (LPN) to join our Hospice Palliative Care Bridge Team. The role focuses on coordinating care, supporting the Palliative Care Nurse Practitioner and Medical Director, and scheduling visits to align with goals of care.

You will document thoroughly, communicate across providers, and assist families as they navigate palliative services in home or facility settings, with top‑of‑market pay and flexible schedules to support your nursing career.

Responsibilities

• Take and process Palliative Care referrals.
• Provide support to the Palliative Care Nurse Practitioner and Medical Director.
• Schedule visits to support goals of care as ordered by MD.
• Coordinate and communicate patient care goals between all providers.
• Make calls to present the palliative program, obtain legal consents, and assess interdisciplinary needs.
• Ensure communication of assessment and reassessment findings to attending physician.
• Complete all documentation accurately and timely per policy.
• Inform palliative team of new admissions and discharges.
• Coordinate referrals utilizing the referral process.
• Serve as a liaison for medical directors and nurse practitioners, enhancing effectiveness and compliance.
• Run billing report and address validation holds/billing errors

Qualifications

LICENSURE, CERTIFICATION, AND EDUCATIONAL REQUIREMENTS:

• Current, active, and unrestricted Registered Nurse (RN) OR Licensed Practical Nurse (LPN)licensure in the state of practice
• A minimum of three (3) years of nursing experience
• Palliative care, hospice, or home health care experience is preferred
• CHPN (Certified Hospice and Palliative Nurse) is preferred
