Residential Home Health and Hospice in Illinois is seeking a Home Health Transitional Nurse Liaison to connect patients and caregivers with our continuum of care. You will gather medical history, introduce services, coordinate start of care, and arrange DME, infusion, and wound care as needed.

This role emphasizes patient education, discharge planning support, and collaboration with facilities to ensure safe returns home.

Qualifications

• RN licensure in good standing required.
• Minimum of one year in an acute care setting.
• Minimum of one year in a customer service capacity.
• Committed to providing high-level customer service to referral sources.
• Ability to multi-task.

Responsibilities

• Obtain complete demographic and medical history, including diagnoses and primary care information.
• Introduce Residential Home Health and Hospice to patient/caregiver and coordinate start of care visit.
• Order DME, infusion services, and wound care as requested.
• Engage in community education with physician offices and facilities (in-services, clinics, health fairs).
• Educate patients at bedside and coordinate follow-up appointments.
• Attend discharge planning meetings and collaborate with facility teams for safe discharge home.
• Evaluate appropriateness of skilled home care services.

Home Health Transitional Nurse Liaison Responsibilities:

• Responsible for obtaining complete and accurate demographic information, medical history including diagnosis for care and primary care physician information.
• Responsible for introducing Company to the patient/caregiver, explaining scope of our services, skilled services requested and coordinating start of care visit.
• Other duties include ordering requested DME, infusion services, and wound care.
• Community Education as requested by our sales to physician offices, assisted living facilities, including but not limited to educational in-services, blood pressure clinics and health fairs.
• Patient education at bedside, arranging post discharge physician follow up appointments, sharing important discharge information with the primary care physician.
• Attending discharge planning meeting as requested, working with the facility team to provide a safe and successful discharge home.
• Determining appropriateness of skilled home care services.

Home Health Transitional Nurse Liaison Qualifications:

• Must have RN Licensure in good standing.
• Minimum of one (1) year of experience in an acute care setting.
• Minimum of one (1) year of experience in a customer service capacity.
• Committed to providing highest-level customer service to our referral sources
• Able to multi-task
