Health Here is seeking a Hospice Clinical Liaison RN to serve as the primary clinical link between referral sources, patients, and the hospice care team. The role provides education to healthcare providers and families about hospice services, assesses clinical eligibility, and coordinates timely admissions.

The ideal candidate will have hospice experience, strong assessment and communication skills, and the ability to collaborate with multidisciplinary teams to support compassionate end-of-life

Qualifications

• RN with hospice liaison experience preferred.
• Strong clinical assessment skills and knowledge of hospice eligibility and care planning.
• Excellent communication, education, and relationship-building abilities.
• Ability to collaborate with multidisciplinary teams and manage complex patient transitions with empathy and professionalism.

Responsibilities

• Partner with physicians, hospitals, and the interdisciplinary team to support patient-centered transition planning.
• Develop plans aligned with patient goals, focusing on symptom management and continuity of compassionate end-of-life care.
• Communicate clearly with referral sources and families to ensure understanding of hospice benefits and care expectations.
• Act as a clinical advocate to promote appropriate referrals and effective admission coordination.
