Residential Home Health and Residential Hospice in Northbrook, IL is seeking a Transitional Nurse Liaison to support our hospice patients and families. You will partner with field staff and leaders to assess needs, coordinate start-of-care visits, and ensure smooth transitions from hospital to home-based care.

The role requires an RN with at least two years of acute care or oncology experience, proficiency with EPIC/HCHB, and strong communication.

Qualifications

• RN with an associate degree and active nursing license.
• Minimum 2 years of experience in acute care or oncology; hospice experience preferred.
• Proficiency with EPIC, HCHB, and MS Office.
• Able to transfer patients using proper body mechanics and ensure patient safety.

Responsibilities

• Complete initial and ongoing patient/family hospice assessments.
• Gather medical history and care needs to determine appropriate services.
• Introduce agency services and coordinate start-of-care visit.
• Order DME, supplies, and medications related to hospice care.
• Deliver community and bedside education on hospice and end-of-life care.
• Attend discharge planning meetings and support safe home transitions.
• Communicate with clinical team to prevent readmissions.
