Job summary

St. Croix Hospice is seeking a Hospice Transition Nurse to serve as a resource RN for patients, families, and hospital staff. The role focuses on sustaining and increasing referral sources from hospitals, nursing homes, and community sources while educating stakeholders about hospice options and services.

The nurse will manage hospital cases, perform goals of care meetings, and coordinate transitions to home or hospice care, ensuring seamless care coordination and adherence to program guidelines.

Qualifications

• Current RN license in state(s) of practice.
• 1+ year of hospice/home care experience preferred.
• Multistate licensure acceptable; maintain RN license in primary state of residence.

Responsibilities

• Act as a case manager for hospital patients, guiding hospice admission and care.
• Conduct goals of care discussions with patients and families in hospital settings.
• Educate hospital staff and referral sources about St. Croix Hospice programs and services.
• Coordinate transitions of care from hospital to home or hospice, including meds and DME needs.
• Mentor and train new RNs for high acuity patient needs.
• Attend interdisciplinary care meetings and maintain hospital relationships with case managers, social workers, and physicians.
