St. Croix Hospice is seeking a compassionate Hospice Transition Nurse to serve as a frontline resource for patients, families and hospital staff.

You will coordinate hospice admissions, educate referral sources, and support hospital discharge planning with a focus on quality of life and dignified end‑of‑life care. The role requires an active RN license in practice states, strong case management and EMR navigation skills, and the ability to travel to hospitals and nursing facilities to advocate

Formación

• Active RN license in state(s) of practice.
• Graduate of a professional nursing program and licensed/certified.
• Ability to pass background check, DHS check, and drug test.

Responsabilidades

• Performs as a case manager for hospital patients, including GIP patient management.
• Acts as a resource to the hospital; navigates EMR to locate documentation for hospice admission.
• Conducts goals of care meetings with patients and families to describe hospice programs.
• Educates facilities and other referral sources about hospice services in collaboration with business development.
• Performs follow‑up meetings and phone calls after information visits.
• Maintains and grows referral sources from hospitals, nursing homes, and community sources.
• Documents visits and calls with patients, families, and referral sources.
• Assists with transition of care from hospital to home or hospice admission, coordinating with admission nurse and Medical Director.
• Mentors and trains new RNs for high acuity patient needs.
• Attends Interdisciplinary Care Meetings regarding patients and families in the hospital.
• Establishes relationships with hospital case managers, social workers, and physicians; participates in daily rounds as needed.
