Job summary

St. Croix Hospice is seeking a Hospice Transition Nurse to guide patients and families through the end‑of‑life journey. The RN serves hospitals, physicians, and nursing homes as a resource and educator about hospice programs and services.

The role focuses on sustaining referral sources, coordinating transitions of care, and mentoring fellow RNs, with travel as required. A current RN license and hospice/home care experience are essential.

Qualifications

• Current RN license in state(s) of practice.
• 1+ year of hospice/home care experience.
• Ability to pass DHS background, consumer report, and drug test.

Responsibilities

• Acts as a case manager for hospital patients and navigates EMR documentation to support hospice admission.
• Conducts initial goals of care meetings with patients and families.
• Educates hospitals and community referrals about hospice programs and services.
• Documents visits and calls with patients, families, and referral sources.
• Coordinates transition of care from hospital to home or hospice, including with admission nurse and Medical Director.
