Job summary

St. Croix Hospice is seeking a Hospice Transition Nurse to coordinate care for patients transitioning from hospital to home or hospice services.

You will educate hospital and community referral sources about hospice programs and collaborate with the care team to ensure seamless care transitions. The role requires an active RN license, at least 1 year in hospice/home care, and willingness to travel as needed.

Qualifications

• Must have a current RN license in state(s) of practice.
• 1+ years of hospice/home care experience.
• Ability to pass DHS background, consumer report, and drug test.

Responsibilities

• Acts as case manager for hospital patients including GIP when appropriate.
• Navigate hospital EMR to locate documentation needed for hospice admission.
• Conducts goals of care meetings with patients and families.
• Educates facilities and referral sources about hospice programs and services.
• Performs follow-up visits and calls from information visits.
• Sustain and increase referral sources with hospitals, nursing homes and community sources.
• Documents visits and phone calls with patients, families, and referral sources.
• Coordinate transition of care from hospital to home or to hospice care, incl. admissions.
• Mentor RNs for high acuity patient needs.
• Attend Interdisciplinary Care Meetings for patients and families.
• Establish relationships with hospital case managers, social workers, and physicians.
