Job summary

Three Oaks Hospice is looking for a Team Manager to coordinate daily operations for an interdisciplinary hospice team, ensuring care aligns with the plan of care and regulatory requirements. This on-site role emphasizes efficient scheduling, accurate EMR documentation, and strong collaboration with physicians and leadership.

You will facilitate IDG meetings, monitor admissions and transitions, and ensure timely, compassionate patient care while upholding Medicare and accreditation standards.

Qualifications

• LPN license or 2+ years healthcare experience.
• Knowledge of hospice philosophy, end-of-life care, and pain/symptom management.
• Ability to work effectively within an IDG.
• Proficiency with EMR systems; Homecare Homebase preferred.
• Understanding of Medicare/Medicaid and accreditation standards (ACHC, CHAP, Joint Commission).
• Knowledge of QAPI programs and performance improvement initiatives.
• Ability to analyze operational and clinical data for continuous improvement.
• Excellent oral and written communication; interact with patients, families, staff, physicians.
• Strong problem-solving and decision-making abilities in a fast-paced environment.
• High level of professionalism, emotional intelligence, and customer service orientation.

Responsibilities

• Coordinate daily hospice team operations per plan of care and regulatory standards.
• Monitor admissions, visits, and transitions of care to maintain speed to care and patient/family satisfaction.
• Oversee team scheduling to ensure equitable caseloads and timely adjustments.
• Track updates to patient records to ensure accuracy and completeness.
• Facilitate IDG meetings and document in the EMR.
• Collaborate with physicians to ensure timely certifications and symptom management.
• Coordinate with external agencies for patient transfers under travel contracts.
• Ensure Face-to-Face visits are scheduled on time and documentation is compliant.
• Run EMR reports to monitor performance and address backlog or issues.
• Serve as EMR workflow owner and escalate deficiencies to leadership.
• Provide EMR training and suggest workflow improvements.
• Ensure compliance with Medicare, state regs, and accreditation standards.
• Participate in QAPI initiatives and support improvement plans.
• Monitor caseload ratios and productivity with leadership.
• Assist leaders with corrective action plans when needed.
• Act as a central communication link between team, physicians, and leadership.
• Escalate patient/family concerns and participate in service recovery.
• Promote accountability, collaboration, and professional growth.
• Perform other duties as assigned.
