Job summary

Three Oaks Hospice is seeking a Director of Clinical Operations to provide strategic leadership and oversee clinical operations across the agency. You will align patient-centered care with regulatory compliance, financial performance, and staff development while collaborating with senior leadership.

You will supervise RNs, LPNs, CNAs, social workers, chaplains, team managers, and IDGs; drive improvements through QAPI, PIPs, and KPI tracking to ensure excellence in hospice care.

Qualifications

• BSN required; Master’s in nursing, healthcare administration, or related field preferred.
• Active RN license required.
• Minimum 2 years of clinical management or supervisory experience in hospice or home health.
• Proficient in EMR systems; knowledge of Medicare/Medicaid and accrediting body standards.

Responsibilities

• Direct overall clinical operations of assigned teams, ensuring regulatory compliance and high-quality care.
• Supervise RN, LPN/LVN, CNA, SW, Chaplains, TM, and IDG members; promote teamwork.
• Ensure policy implementation and readiness for surveys and accreditation.
• Lead QA/QI initiatives (QAPI, PIPs) and conduct clinical data analysis for improvements.
• Develop staffing models and manage budgets, productivity, and KPIs across teams.
