Job summary

The Team Manager is responsible for maintaining patient records, supervising staff, and ensuring regulatory compliance while coordinating 24/7 care and on-call coverage.

Qualifications

• Bachelor's degree preferred; RN license required.
• Two years supervisory experience in a health care setting.
• Knowledge of primary medical and nursing care and bereavement processes.
• Working knowledge of Vx and ability to use management reports.

Responsibilities

• Directs all patient care services for the team as integrated components of the plan of care.
• Collaborates with team physician and attending physician on palliative care measures.
• Ensures accurate documentation and timely recertification decisions.
• Monitors team resources and staffing to meet patient/family needs.
• Manages the team schedule and coordinates 24/7 patient care coverage.
• Participates in on-call rotation and team meetings.
