Job summary

Bachelor's degree and RN license are preferred; supervisory experience in healthcare is required or equivalent.

Qualifications

• Knowledge of the principles and practices of primary medical and nursing care, with at least a working knowledge of oncologic nursing and palliative care.
• Two years successful supervisory experience or equivalent in a health care organization.

Responsibilities

• Directs all patient care services for the team, including volunteer and bereavement, and ensures integrated care.
• Reviews information on pending admissions and alive discharges to confirm eligibility and appropriateness.
• Collaborates with the team physician and attending physician on palliative care measures for pain and symptom control.
• Assures complete data and documentation for recertification decisions and timely updates.
• Manages team schedule and coordinates services 24/7 for all patients on the team.
• Facilitates team meetings to update the care plan with interdisciplinary input and family needs.
• Participates in on-call rotation.
