Job summary

Tranquility Hospice is seeking a dedicated STNA/Hospice Aide to provide hands-on, compassionate care under the supervision of the RN/Case Manager. You will follow the patient’s Hospice Aide Plan of Care and ensure accurate documentation of services, observations, and changes during visits.

This full-time role requires local travel throughout Delaware, Marion, and surrounding areas. You will assist with personal care, activities of daily living, observation, and family support while maintaining

Qualifications

• Successful completion of NATCEP and competency requirements.
• Current listing in good standing on the Ohio State Nurse Aide Registry.
• Minimum of one year STNA/nurse aide experience in hospice or related setting preferred.
• Hospice experience preferred.
• Strong attention to detail and ability to document accurately.

Responsibilities

• Follow the assigned Hospice Aide Plan of Care and instructions from the supervising RN/Case Manager.
• Provide personal care and aid with activities of daily living.
• Complete all required documentation accurately and promptly.
• Observe and report changes in patient condition to the RN/Case Manager.
• Travel locally to assigned service areas and support patients in homes and facilities.
