Job summary

Mercy Medical Center Home Health and Hospice is seeking a Hospice Aide I on a PRN basis to provide compassionate end-of-life care under RN supervision. You will assist with personal care tasks, support patients and families, and help maintain comfort in home settings.

Responsibilities include ADLs support, noting patient changes, documenting care, infection control, and safeguarding patient confidentiality per HIPAA guidelines.

Qualifications

• Current certification as a Nurse Aide (CNA, STNA, or equivalent) in the state of practice.
• Minimum of 1 year of experience providing personal care in hospice, home health, or long-term care preferred.
• Ability to read, write, and communicate effectively in English.
• Valid driver's license, reliable transportation, and current auto insurance required.

Responsibilities

• Provide personal care to patients in accordance with the plan of care and under the supervision of the RN.
• Assist with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and mobility support.
• Offer emotional and physical comfort to patients and caregivers during end-of-life care.
• Observe and document patient condition, changes, and care provided; report changes to the RN.
• Maintain a clean and safe environment for the patient, including light housekeeping related to patient care.
• Use infection control techniques and universal precautions consistently.
• Provide support to the patient and family within their cultural and personal belief system.
• Maintain confidentiality and comply with all HIPAA, company, and regulatory guidelines.
• Participate in interdisciplinary team meetings and in-service education as required.
• Maintain accurate documentation in accordance with agency standards.
• Perform other duties as assigned.
