Job summary

Reserve Care is seeking a Part-time STNA/CNA for our Westlake inpatient facility, day shift. The role involves providing personal care under nurse supervision according to the patient’s plan of care and collaborating with a transdisciplinary team in a variety of settings.

Responsibilities include assisting with ADLs, maintaining a safe patient environment, assisting with transfers and ROM, helping with nutrition, and documenting activities and observations.

Qualifications

• Must be able to read and write English proficiently.
• Must be state tested nursing assistant in good standing.

Responsibilities

• Provide personal care and assist with ADL's according to the POC.
• Provide safe environment for patients.
• Safely ambulate/transfer patient as appropriate.
• Assist with passive and active ROM exercises as directed by the nurse.
• Prepare nutritional snacks as appropriate for the patient.
• Under supervision, assist with pre-poured medications (Home Care only).
• Perform simple wound care, skin care, and bowel care protocols as directed.
• Record in clinical record, on appropriate documents, activities and observations.
• Complete all charting, timesheets and mileage logs as per policy.
• Attend patient care conference and meetings as appropriate.
• Assist with coordinating care and communication with staff and volunteers.
