Job summary

Reserve Care seeks a STNA/CNA for Westlake Inpatient Facility Day Shift. Under nurse supervision, provide personal care and assist with ADLs per care plan, ensure safety, and support a compassionate multidisciplinary team in a hospice setting.

You will document care, participate in chart reviews, and contribute to quality improvement while maintaining transmission-based precautions and patient comfort. This role offers full-time hours with weekend flexibility.

Qualifications

• Must be a state tested nursing assistant in good standing.
• Provide personal care under nurse supervision according to the patient plan of care.
• Maintain patient safety and observe changes in condition.

Responsibilities

• Provide personal care and assist with ADLs per POC.
• Assist with safe ambulation and transfers as needed.
• Support ROM exercises as directed by the nurse.
• Prepare nutritional snacks for patients.
• Record activities and observations in clinical records.
• Attend patient care conferences and meetings.
