Hospice of the Western Reserve, Inc. in Westlake, Ohio, is seeking a STNA/CNA for the inpatient facility on day shift.

This regular part-time role works under nurse supervision to provide personal care per the patient's plan of care and to support a compassionate, multidisciplinary team. Responsibilities include assisting with ADLs, safe patient handling, basic wound and skin care, reporting changes, and documenting observations.

Qualifikationen

• Must be a state-tested nursing assistant in good standing.
• Excellent English reading and writing skills.
• Previous hospice or inpatient care experience is a plus.

Aufgaben

• Provide personal care and assist with ADLs according to the plan of care.
• Maintain a safe patient environment and assist with transfers as needed.
• Support with ROM exercises and nourishment as directed.
• Assist with pre-poured medications under nurse supervision (Home Care only).
• Document activities and observations in the clinical record during visits.
• Participate in team meetings and care conferences.
