Reserve Care is seeking a licensed CNA to provide personal care under nurse supervision in home care settings. You will collaborate with the care team to support patient plans and comfort needs while ensuring safety and quality of life for patients and families.

The role emphasizes integrity, communication, and adaptability, with participation in meetings and ongoing competency requirements as part of a compassionate care team.

Qualifications

• Must be able to read and write English proficiently.
• Must be state tested nursing assistant in good standing.
• Equivalent education/experience may substitute for minimum qualifications except when there are legal requirements, such as a license/certification/registration.

Responsabilités

• Provide personal care under the supervision of a nurse according to the patient's plan of care.
• Collaborate with transdisciplinary team in a variety of settings.
• Provide a safe environment for patients.
• Safely ambulate/transfer patient as appropriate.
• Assist with passive and active ROM exercises as directed by the nurse.
• Under supervision, assist with pre-poured medications for Home Care only.
• Perform simple wound care, skin care, and bowel care protocols as directed.
• Record activities and observations in the clinical record.
