Job summary

Crossroads Hospice & Palliative Care seeks a dedicated STNA (CNA) to join our team in Ohio. You will provide compassionate bedside care, collaborate with an interdisciplinary team, and support patients’ families throughout the care journey.

The role requires Ohio STNA certification, active certification, and at least 1 year of hospice or home health aide experience. Travel is involved, with a Monday–Friday schedule in Green, OH.

Qualifications

• Must have completed an Ohio STDH Competency Test with 75+ hours of certification.
• Must be certified by the state as a nurses' aide with an unrestricted, active aide certification.
• 1 year of Hospice or Home Health Aide experience required.
• Hospice or Home Health Agency experience preferred.
• Interpersonal skills to interact with patients, families, and the care team.
• Ability to learn computer skills related to clinical software and EHR utilization.

Responsibilities

• Provide hospice or palliative care at the patient’s bedside with dignity and compassion.
• Collaborate with an interdisciplinary care team to plan and deliver care.
• Support patients’ families, answer questions, and provide updates on care plans.
• Travel to the Green, OH office as part of the schedule and responsibilities.
