Job summary

Ohio is seeking a Personal Care Specialist (CNA/STNA) to provide hands-on home-based patient care in Wooster, OH. The role involves working with nurses, families, and the hospice team to deliver compassionate, dignified support in the patient’s home.

Schedule is full-time, Monday–Friday, 8:00 a.m.–4:30 p.m., with travel across Wayne, Summit, and Medina counties. A valid Ohio STNA/CNA certification and basic computer skills are required or preferred.

Qualifications

• High school diploma or GED required.
• Current Ohio STNA/CNA certification preferred.
• At least one year of healthcare or direct patient-care experience preferred.
• Basic computer skills and ability to use electronic documentation systems.

Responsibilities

• Assist patients with activities of daily living (ADLs) such as bathing, dressing and mobility.
• Provide compassionate, respectful, patient-centered care.
• Promote comfort, safety, privacy, dignity and independence.
• Observe for changes in condition and promptly communicate concerns to the care team.
• Accurately document care and services using electronic documentation systems.
• Communicate effectively with nurses, caregivers, family members and hospice team.
