Job summary

Transitions Hospice in Worthington, OH is seeking a compassionate CNA to join our interdisciplinary care team. You will provide in-home care to patients, delivering personal support with dignity while documenting vitals and observations.

This role covers Franklin County and surrounding areas; schedule Mon-Fri, 8-hour shifts; pay range $20–$25 per hour with benefits including health insurance, disability, 401(k), and PTO.

Qualifications

• Active CNA license or certification in the state of employment.
• Current CPR certification required.
• One year or more hospice or home health experience preferred.

Responsibilities

• Providing personal care with dignity, comfort, and respect at the center of everything.
• Attending to patient needs and requests promptly and compassionately.
• Monitoring and recording vital signs (temperature, pulse, respiration, blood pressure) as directed.
• Observing patients and reporting changes to the care team accurately and in a timely manner.
• Communicating with patients and families in a positive, ethical, and confidential manner.
• Maintaining thorough documentation in line with organizational standards.

What You'll Do

Requirements

What You Bring

• Active CNA License or Certification in the state of employment
• Possess and maintains current CPR certification
• One (1) + year Hospice, home health or related organization experience preferred
• Comfort and sensitivity working with terminally ill patients and their families
• A calm, dependable presence under emotionally demanding circumstances
• Strong attention to detail in observation and documentation
• A valid driver's license and willingness to travel locally for patient visits
