Transitions Hospice - Ohio is seeking a compassionate CNA to join our interdisciplinary care team, providing in-home, dignified care to patients in Franklin County and surrounding areas. The schedule is Mon-Fri, 8-hour shifts, with a pay range of $20.00-$25.00 per hour.

As a CNA on our team, you will deliver personal care, monitor vital signs, and communicate with families and the care team, while maintaining thorough documentation and upholding our hospice philosophy.

Qualifications

• Active CNA license in state of employment.
• Current CPR certification.
• 1+ year hospice, home health, or related experience preferred.
• Comfort with terminally ill patients and families.
• Calm, dependable presence under emotionally demanding circumstances.
• Valid driver’s license and ability to travel locally.

Responsabilités

• Provide personal care with dignity, comfort, and respect at the center of everything.
• Attend to patient needs and requests promptly and compassionately.
• Monitor and record vital signs (temperature, pulse, respiration, blood pressure) as directed.
• Observe patients and report changes to the care team accurately and in a timely manner.
• Communicate with patients and families in a positive, ethical, and confidential manner.
• Maintain thorough documentation in line with organizational standards.
• Uphold the hospice philosophy with professionalism and genuine compassion.

Requirements

What You Bring

• Active CNA License or Certification in the state of employment
• Possess and maintains current CPR certification
• One (1) + year Hospice, home health or related organization experience preferred
• Comfort and sensitivity working with terminally ill patients and their families
• A calm, dependable presence under emotionally demanding circumstances
• Strong attention to detail in observation and documentation
• A valid driver's license and willingness to travel locally for patient visits
