Job summary

Transitions Hospice is seeking a compassionate Certified Nursing Assistant to provide in-home care across Rantoul, Champaign, Piatt, Paxton and nearby areas. You will work Mon-Fri, 8-hour shifts, delivering dignified personal care and monitoring patients in their homes.

As part of an interdisciplinary team, you’ll record vital signs, respond to requests promptly, and communicate with families in a respectful, confidential manner. Competitive pay and comprehensive benefits complete the package.

Qualifications

• Active CNA license or certification in the state of employment.
• CPR certification is required.
• One year hospice, home health or related experience preferred.
• Comfort with terminally ill patients and families.
• Calm, dependable presence under emotionally demanding circumstances.
• Strong attention to detail in observation and documentation.
• Valid driver’s license and willingness to travel locally for patient visits.

Responsibilities

• Providing personal care with dignity, comfort, and respect at the center of everything.
• Attending to patient needs and requests promptly and compassionately.
• Monitoring and recording vital signs (temperature, pulse, respiration, blood pressure) as directed.
• Observing patients and reporting changes to the care team accurately and in a timely manner.

• Maintaining thorough documentation in line with organizational standards.
• Upholding the hospice philosophy with professionalism and genuine compassion.

Schedule: Mon-Fri (8 hour shifts)

Pay Range: $19.00 - $24.00 an hour

What You Bring

• Active CNA License or Certification in the state of employment

• One (1) + year Hospice, home health or related organization experience preferred
