Transitions Hospice is seeking a caring CNA to join our team in Champaign and surrounding areas. You will provide in-home personal care, monitor vital signs, and support families with compassion and professionalism.

Responsibilities include documenting observations, communicating with the care team, and upholding hospice philosophy. A CNA license and CPR certification are required, with travel to local patient visits expected.

Formación

• Active CNA license or certification in the state of employment.
• Current CPR certification.
• One (1) year+ hospice/home health or related experience preferred.
• Comfort and sensitivity when working with terminally ill patients and families.
• Calm, dependable presence under emotionally demanding circumstances.
• Strong attention to detail in observation and documentation.
• Valid driver's license and willingness to travel locally for patient visits.

Coverage Territory: Champaign, Piatt and surrounding areas

Schedule: Mon-Fri (8 hour shifts)

Pay Range: $19.00 - $24.00 an hour

What You'll Do

As a Transitions Hospice CNA, you're a vital member of the interdisciplinary care team — not just a support role, but a trusted presence for patients and families. Your responsibilities include:
