Job summary

Transitions Hospice is seeking a compassionate CNA to join our home-based care team in Pekin, IL. You will provide patient-centered, dignified personal care in patients' homes as part of an interdisciplinary team.

Responsibilities include vital signs, timely documentation, and compassionate communication with families. A valid CNA license and CPR are required, with hospice/home health experience preferred.

Qualifications

• Active CNA license in the state of employment.
• Current CPR certification.
• 1+ year Hospice/home health or related experience preferred.
• Comfort with terminally ill patients and families.

• Providing personal care with dignity, comfort, and respect at the center of everything.
• Attending to patient needs and requests promptly and compassionately.
• Monitoring and recording vital signs (temperature, pulse, respiration, blood pressure) as directed.
• Observing patients and reporting changes to the care team accurately and in a timely manner.
• Communicating with patients and families in a positive, ethical, and confidential manner.
• Maintaining thorough documentation in line with organizational standards.
• Upholding the hospice philosophy with professionalism and genuine compassion.

Pay Range: $19.00 - $23.00 an hour

Requirements

What You Bring

• Active CNA License or Certification in the state of employment


• Possess and maintains current CPR certification


• One (1) + year Hospice, home health or related organization experience preferred


• Comfort and sensitivity working with terminally ill patients and their families


• A calm, dependable presence under emotionally demanding circumstances


• Strong attention to detail in observation and documentation


• A valid driver's license and willingness to travel locally for patient visits
