Job summary

Care Initiatives Hospice is seeking a Hospice Spiritual Care Coordinator to provide compassionate spiritual care, meeting patients where they are—at home, in long-term care, assisted living, or hospital settings.

You will assess spiritual needs, collaborate with families and caregivers, and participate as a member of the interdisciplinary team to integrate spiritual care into the patient’s plan. Competitive benefits and ongoing training accompany this role.

Qualifications

• Master of Divinity degree from an accredited seminary and ordination or licensure as applicable.
• Baccalaureate degree in social work (MSW supervision) or in psychology/sociology/counseling related to pastoral care or grief services.

Responsibilities

• Arrange counseling to meet spiritual needs of patients/families/caregivers.
• Provide comprehensive assessment of patient, family, and caregiver’s spiritual needs and bereavement needs.
• Identify spiritual and bereavement interventions to support the care plan.
• Collaborate with the interdisciplinary team to ensure services align with the patient’s care plan.

Education
