Job summary

Care Initiatives Hospice is seeking a compassionate Spiritual Care Coordinator to provide holistic end-of-life support at home, in care communities, or hospitals. You will assess spiritual needs, offer counseling, and collaborate with the IDT to honor patient beliefs and wishes.

As a part-time role, you’ll meet families where they are, coordinate care plans, and deliver bereavement support with dignity and respect during life’s final journey.

Qualifications

• Master of Divinity Degree from an accredited seminary; ordination or licensure as applicable, endorsed in good standing by the ecclesiastical governing body.
• Bachelor's degree in social work (BSW) with MSW supervision pathway or equivalent.
• Bachelor's degree in psychology, sociology, or counseling or related field to pastoral care, grief services, or counseling.

Responsibilities

• Arrange for counseling to meet spiritual needs of patients, families, and caregivers in line with their beliefs.
• Provide comprehensive assessment of patient, family, and caregiver's spiritual needs; complete bereavement risk assessments and determine bereavement needs.
• Identify spiritual and bereavement interventions and goals for the patient’s care plan.
• Participate as a member of the interdisciplinary team (IDT) in developing the patient care plan and ensure services align with patient acceptance and care plan.
