Job summary

Vitality Hospice & Palliative Care is seeking a Spiritual Care Coordinator (Chaplain) for per diem, on-the-road care. You will deliver compassionate, patient-centered spiritual care to hospice patients, families, and caregivers, collaborating closely with the IDT to ensure holistic, culturally sensitive care aligned with hospice philosophy and regulatory standards.

Qualifications

• Graduate of an accredited seminary or school of theology (B.D., M.Div., or equivalent), or degree in counseling, or equivalent experience.
• Experience working with death and dying.
• Ability to work effectively within an interdisciplinary team.
• Comfort working in an interfaith, multicultural environment.
• Knowledge of and commitment to hospice philosophy.

Responsibilities

• Assess spiritual needs and provide supportive spiritual care to patients and families.
• Develop, implement, and document individualized spiritual care plans.
• Collaborate with the Interdisciplinary Team to coordinate holistic care.
• Participate in IDT meetings and care plan reviews.
• Provide consultation, education, and support to staff regarding spiritual care.
• Coordinate with clergy, faith leaders, and community spiritual resources as requested.
• Conduct pre-bereavement and bereavement assessments, visits, and follow-ups.
• Facilitate or conduct funeral and memorial services for patients, families, facilities, and staff as requested.
• Network with faith communities to educate on hospice philosophy.
• Supervise spiritual care volunteers as assigned.
