The position requires a Master’s in Social Work with NY licensure, a valid driver’s license, and relevant healthcare social work experience.

Qualifications

• Master's degree in Social Work and NY licensure required.
• Valid driver’s license and vehicle required for on-call duties.
• Relevant clinical social work experience in health care, hospice, or palliative care preferred.

Responsibilities

• Coordinate patient care and triage social work needs.
• Provide counseling, advocacy and community resource coordination.
• Maintain timely documentation and HIPAA compliance.
• Participate in on-call and holiday rotation.
