Job summary

Community Care of WNY is seeking a dedicated Family Support Specialist to deliver direct social services to patients and families under supervision of a licensed social worker. This role requires counseling, case management, and collaboration with the care team to support care plans and bereavement needs.

Responsibilities include developing plans of care, coordinating with social and community resources, and maintaining accurate documentation.

Qualifications

• Bachelor's degree in Social Work or related field plus 1 year of paid social work experience or Bachelor’s in Human Services with 2 years of paid social work experience.
• Experience with terminally ill patients and families preferred.
• Demonstrated organizational and supervisory skills.

Responsibilities

• Establishes plans of care/objectives for patients and families in consultation with supervisor.
• Provides counseling and/or case management to patients and families per plan of care.
• Assists team in understanding social and emotional factors during illness, bereavement, and care.
• Participates in interdisciplinary meetings to discuss plans of care and revisions.
