Elite Hospice, a part of LHC Group, is seeking a Social Worker to join our interdisciplinary team under physician supervision. The role focuses on understanding social and emotional factors affecting patient health and developing coping strategies to support families.

Qualifications

• Master's Degree from a CSWE-accredited school.
• 1+ years of social work experience in a health care setting.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.

Responsibilities

• Assists patient, family, physician and health care team to understand social and emotional factors related to health status.
• Contributes to development of a comprehensive Plan of Care on a daily basis.
• Instructs team on community resources available to assist patients as needed.
• Function as Bereavement Coordinator and supervise bereavement services with a clear plan of care.
