Job summary

University of Rochester Medicine Home Healthcare is seeking a Hospice Social Work Manager to provide leadership, supervision, and clinical oversight of hospice social work services in the Rochester area.

The role requires an MSW from a CSWE-accredited program, NY LMSW/LCSW license, five years of clinical social work experience, and at least two years in a supervisory role. Schedule is Monday–Friday, 8:00 AM to 4:30 PM, with responsibilities across CHHA and interdisciplinary teams.

Qualifications

• Master's degree in social work from a CSWE-accredited program.
• Minimum five years of clinical social work experience; hospice/home care/ palliative care preferred.
• At least two years of supervisory or clinical leadership experience.

Responsibilities

• Provide daily leadership, supervision, and clinical guidance to Hospice social work staff.
• Manage direct reports through supervision, coaching, performance appraisal, payroll approval, and time-off approval.
• Participate in recruitment, onboarding, competency assessment, and retention of Hospice social work staff.
• Assign and balance Hospice caseloads based on patient need, census, geography, and staff capacity.
• Monitor productivity, timeliness, documentation quality, patient access, and performance indicators; address barriers as needed.
• Ensure Hospice social work practice complies with federal/state requirements and Medicare Hospice Conditions of Participation.
• Provide clinical consultation for psychosocial assessment, care planning, crisis response, and discharge planning.
• Maintain a small Hospice caseload and provide field coverage when needed.
• Develop and maintain Hospice social work policies, procedures, and workflows.
