Job summary

Hospice Buffalo is seeking a Medical Social Worker-Triage Coordinator to support daily clinical operations, coordinate patient care, and liaise with staff, families, and external partners. The role requires strong clinical awareness and the ability to maintain office processes and timely documentation.

You will triage social work and spiritual care needs, assist with training and department development, prepare reports, and participate in on-call and holiday rotations.

Qualifications

• Master's degree in Social Work from an accredited school with current licensure in NY.
• Valid Driver's License, free of serious violations, and vehicle required for use on duty or on-call.

Responsibilities

• Provides skilled social work services including counseling, support, advocacy, long term planning, community resource coordination, crisis intervention and other services related to the terminal illness that must be addressed to promote the patient's well-being, comfort, and dignity throughout the dying process to optimize patient care.
• Observes, records, and reports the patient and family's reaction to treatment and any change in the patient's condition to the physician and/or patient case manager and other staff involved.
• Assists the physician and other members of the home health team to understand the significance of social and emotional factors.
• Assess and reassess social and emotional factors to help the patient and family to cope with problems in daily living and to help them follow medical recommendations to maximize quality of life.
• Execute patient/family/caregiver teaching in all given situations.
• Adheres to Social Work Quality Assurance standards, keeps abreast of social work trends and knowledge for service provision; and assists in audits/peer reviews as directed by his/her supervisor.
• Provides timely documentation/data entry per Hospice time frames and deadlines.
• Adheres to requirements of both CHPC and Hospice Buffalo policies and procedures that are both patient focused (clinical) and organizational focused (administrative); Adheres to HIPPA regulations and policies.
• Maintains the security of patient electronic/written records.
